To complete your scholarship application

  • Click on the scholarship application button below, review, and complete

  • Use the letter template below to submit your request to the DHHS Licensure Unit for an academic review to verify you have completed the pre-mortuary academic requirements.

  • Complete a personal statement. You will include in the scholarship application

  • Ask a member of NeFDA to endorse your application and write a statement of recommendation

Applications must be date stamped/postmarked by June 15 to be eligible

A completed application should include:

  1. Completed application form

  2. Transcripts for high school and college

  3. Letter from DHHS showing academic review completed

  4. Statement of recommendation from a NeFDA member

After the scholarship deadline, you will receive a notification your application was received. A personal interview will be scheduled with the Scholarship Committee in mid-July. Details of the interview will be emailed to you once the interview date is confirmed. Applicants must appear in person before the committee, no exceptions, for your eligibility to be met.
